To fill out the intake form:
- Click here to open the PDF (it will open in a new window)
- In the top right corner of the browser, click the down arrow to download the PDF to your computer
- Open the PDF from wherever you saved it (typically a “Downloads” folder)
- Either print and fill out the fields, or you can fill it out electronically via the editable fields
- Save the updated version and email it as an attachment to Julie at colt_co@icloud.com
